In phase two, place-based planning with three or four prioritised communities will co-design, implement and evaluate new models of care. Research will be undertaken to evaluate the workings of the co-design process with each community, the outcomes delivered (what works in which contexts, for whom and how), and the impact of these on local and regional health service provision and in terms of economic impact. Impacts and outcomes of the project will be evaluated in terms of quality of care, efficiency, service provider and consumer satisfaction, and process acceptability and sustainability.
